Home & Community Based Program

Brookville Center's Home-Community Based Program offers therapeutic and educational services to children in their own home, nursery school, preschool or other natural environments.

The program offers Early Intervention services to children birth to 3 years of age.  Upon referral from the Department of Health, families may receive a CORE evaluation in their home.  If their youngster qualifies, services can be provided in the home.

Preschool children 3-5 years of age may receive Special Education Itinerant services (SEIT) and/or Related Services in their home or school.  This service must be determined by the child’s local school districts Committee on Preschool Special Education (CPSE).
Upon referral from the child's residing school district, school age students can receive Educational Services, Behavioral Services and/or Related Services within their own home as well as at an alternative community-based site.  In home parent training is also available.

Enrollment is determined by referral from the Department of Health for Early Intervention Services and from the family's residing school district for preschool and school age children.

For more information about our Home & Community Based Program, please contact Dr. Subashini Jesurasa, (516) 626-1075 x2498.

Our mission is to provide comprehensive services to children with an emphasis on assisting families in understanding their child's strengths and areas of need.
