Transition Services/Supports at BCCS
We are dedicated to guiding and supporting you and your child through every step of their educational journey. We know that transitions can be challenging, and our supports are designed to provide seamless assistance across every stage and into adulthood.
Early Intervention (EI) Transition to Preschool
BCCS is here to assist children in their transition from Early Intervention Services (EI) to Preschool Special Education.
Children receiving Early Intervention services may need required support through Preschool. An additional referral from your local school district’s Committee on Preschool Special Education (CPSE) is required for your child to receive Preschool Special Education services after Early Intervention ends.
Transition to School-Age Services
After Preschool, your child may need additional services for their school-age years. A separate referral from the child’s local school district’s Committee on Special Education (CPE) is required for your child to receive Special Education services after Preschool.
BCCS supports students transitioning from Preschool Special Education to School Age Special Education.
Providing an Individualized Transition Plan to Adult Services
Many children with developmental disabilities and their families question what kinds of supports are available when school ends and life as an adult begins. BCCS assists parents with an individualized transition plan for their child upon completing school and transitioning to adult services.
We strongly recommend that transition planning for post-secondary school life start at age 14, but it’s never too early to research all possibilities and options. This could involve transitioning into Adult Services or embarking on your own, or with the support of family or a caregiver. We know this is an emotional and overwhelming phase for parents, and we’re here to help prepare you and your child for the future.
Transition to Adulthood and Beyond
All BCCS adolescent students can access our services, which include pre-vocational training at community sites.
Pre-vocational training is a service offered to all BCCS students at community-based sites. Activities are based on the child’s individualized transition plan, considering their abilities, preferences, and interests.

Contact Our Central Enrollment Department
We will assist you with the following:
- Registering your child, determining eligibility, and scheduling a time to attend a Front Door Access to Services Information Session for Individuals and Families.
- Discover resources and assistance options to empower your family members. (i.e., Self-Directed Services, Community Habilitation, day options, living options, etc.).
- Obtain Social Security Income (SSI) and Medicaid for additional financial and health care support.
